DOT Physicals and Pre-Employment/Fit for Duty Tests

DOT Physicals and Pre-Employment Physicals Fees:

Driver physical only: $80.00
Driver Physical with CDL self-certification: $100.00
Drug Tests: $55.00
Pre-employment/Fit for Duty Tests: $85.00

Required for DOT Physicals

Drivers will need:
• Driver’s License
• Glasses if needed for eye exam
• List of current medications
• If Diabetic: Most recent Hemoglobin A1C
• If Diabetic with insulin use: The Insulin-Treated Mellitus Assessment Form filled out by treating
clinician (Please have driver mention if they are on insulin so this form can be sent to their treating
doctor in advance of their appointment.)
• If CPAP used for Sleep Apnea: CPAP Usage Report
• If recent surgery or injury: Clearance from attending physician
• If followed by cardiologist: Last visit office note from cardiologist.
• Be prepared to provide a urine sample. This is a screening for blood, protein and sugar only
